Privacy Policy

Polaris Dental Group

Last updated: September 29, 2026

ABOUT THIS POLICY

This policy explains how Polaris Dental Group ("we", "us", "the practice") handles

information collected through our website, by text message and by email.

Your health information is covered separately. Dental records and other protected

health information are governed by our Notice of Privacy Practices under the Health

Insurance Portability and Accountability Act (HIPAA), which is available at the

practice and on request. Nothing in this policy limits the rights that notice gives you.

INFORMATION WE COLLECT

Information you give us. Your name, phone number, email address and anything you write

when you contact us through the website, by text or by email - for example to ask about

an appointment.

Information collected automatically. Like most websites, ours records basic technical

information such as your browser type, device and the pages you visit. This is used to

keep the site working and to understand which pages people find useful.

We do not ask for your Social Security number, financial account details or health

information through this website.

HOW WE USE IT

- To answer your questions and to contact you about your care and your appointments.

- To send appointment confirmations and reminders, where you have agreed to receive them.

- To keep our website working properly and secure.

- To meet our legal and professional obligations.

TEXT MESSAGES

Consent. We send text messages only to patients who have agreed to receive them. You may

agree on our patient form, in person at the practice, or by telling a member of our team

on the phone. Agreeing is optional and is never a condition of receiving dental treatment.

What we send. Appointment confirmations and reminders, notices that you are due for a

routine visit, and occasional offers of an earlier appointment time that has become

available. You can reply to any message and a member of our team will answer. We do not

send advertising, and our messages do not contain details of your treatment.

How often. Around 2 to 6 messages a month, depending on how often you visit. Message and

data rates may apply, depending on your mobile plan.

Stopping messages. Reply STOP to any message and we will stop texting you. Reply HELP for

help, or call us at 770-517-2688]. You can also ask us in person or by phone at any time.

Stopping text messages does not affect your care - we will still contact you by phone or

post where we need to.

EMAIL

We send email only to patients who have given us an address and agreed to hear from us

that way. You can ask us to stop at any time. Where we send a billing statement by email,

the statement is attached as a document rather than written into the body of the message.

COOKIES AND WEBSITE ANALYTICS

Our website uses cookies and similar technologies to keep the site working and to measure

how it is used. You can block or delete cookies in your browser settings, though some

parts of the site may then not work as intended. We do not use cookies to build

advertising profiles.

HOW WE SHARE INFORMATION

We do not sell or share your SMS opt-in data or personal information with third parties for marketing purposes.

No mobile information is shared with third parties or affiliates for marketing or

promotional purposes. We do not sell, rent or trade your phone number, email address or

any other personal information.

We share information only:

- with service providers who work on our behalf - for example the provider that delivers

our text messages, or our practice management and payment systems - under written

agreements that permit them to use your information only to provide that service and

  require them to protect it;

- with your dental insurance carrier, where needed to obtain benefits or submit a claim;

- where the law requires it, or to protect the safety of a person.

SECURITY

We protect personal information with administrative, physical and technical safeguards,

including encryption of records in our practice systems and restricted access limited to

staff who need it for their work. No system can be guaranteed completely secure, but we

take this seriously and review it regularly.

CHILDREN

Our website is not directed at children, and we do not knowingly collect personal

information from children through it. Where we treat a child as a patient, we deal with a

parent or legal guardian.

HOW LONG WE KEEP INFORMATION

We keep information for as long as we need it to provide your care and for as long as

Georgia law and our professional obligations require dental records to be retained.

YOUR CHOICES

You may ask us at any time to stop sending you text messages or email, to correct

information we hold about you, or to tell you what we hold. For dental records

specifically, the rights set out in our Notice of Privacy Practices apply.

CHANGES TO THIS POLICY

If we change this policy we will update the date at the top of this page.
